Santa Clara County Public Health COVID-19 Vaccine Update
Starting March 15, people age 16-64 who have a qualifying severe health condition or disability or illness became eligible for vaccine in Santa Clara County and across the state.
Physicians do not need to provide letters or other documentation of diagnosis to their patients who have appointments with the County Health System. As recommended by the state of California, vaccine sites operated by the County Health System use a self-attestation approach to verifying eligibility based on health condition or disability. This includes sites such as Levi’s Stadium, the County Service Center on Berger Drive in San Jose, the Fairgrounds, Gilroy High School, Mountain View Community Center, and more. Self-attestation means that people who are eligible are asked to sign a document indicating this. This document is an attestation under penalty of perjury. We make the attestation documents available online, if your patients want to review them in advance.
A person also needs to show that they live or work in Santa Clara County. County Health System sites accept any reasonable documentation that someone lives or works in SCC. You can find a list on our vaccine webpage at sccfreevax.org. People who have health insurance are also asked to bring their health insurance card. We bill that health insurance. There is no co-pay or cost to the patient.
Who is eligible now?
- People who live in Santa Clara County are eligible if they are:
- 65 or above
- A resident of a long-term care facility
- A resident of a congregate residential setting or unhoused or
- 16-64 and have a qualifying severe health condition or disability or illness.
- People who live or work in Santa Clara County are eligible if they work in:
- Healthcare
- Education and childcare
- Emergency services
- Food and agriculture
- A congregate residential setting or
- Public transit
